BACKGROUND: Shoulder pain is a disabling musculoskeletal disorder worldwide. Thus, it is important to identify interventions able to improve pain and disability. OBJECTIVE: To investigate the effects of mobilization with movement (MWM) on pain, disability, and range of motion in patients with shoulder pain and movement impairment. METHODS: A systematic search of different databases was performed. The systematic review protocol has been registered in PROSPERO (CRD42023404128). A random-effects model for meta-analysis was used to determine the mean difference (MD), standardized mean differences (SMD), and 95% confidence interval for the outcome of interest. RESULTS: Twenty-six studies were included. Of these, eighteen were included in the meta-analysis. MWM improved pain during movement with a moderate effect SMD of (-0.6; 95% confidence interval, -1.1 to -0.1, I2 = 0%; N = 66;) and shoulder abduction MD of (12.7°; 1.3 to 24.0; I2 = 73%; N = 90) compared to sham MWM in the short term (0-6 weeks). Combined MWM and conventional rehabilitation improved pain at rest, with a MD of (-1.2; -2.2 to -0.2; I2 = 61%; N = 100), and disability SMD of (-1.3; confidence interval -2.2 to -0.4; I2 = 87%; N = 185) compared to conventional rehabilitation alone in the short term. Combined MWM and conventional rehabilitation also resulted in improvement in shoulder abduction and external rotation. Compared to Maitland, MWM resulted in improvement in the shoulder abduction MD (20.4°; confidence interval 4.3 to 36.5; I2 = 89%; N = 130) in the short term. There is no information regarding long-term effects. CONCLUSION: Evidence suggests that MWM may reduce shoulder pain and restore shoulder range of motion and function. Our findings are promising, but the evidence is not strong enough to recommend it pragmatically.

ABSTRACT Objective: To determine the level of scientific information of dental surgeons who carry out their professional activities in Brazil about antiresorptive drugs and indicated pharmacological procedures aiming at the prevention of osteonecrosis of the jaws and the therapy of drug sequelae that may occur, considering the time since graduation in Dentistry. Material and Methods: This is a quantitative cross-sectional study in which 339 dentists were consulted using the virtual questionnaire containing topics of personal nature, elements contained in the anamnesis carried out and knowledge about antiresorptive drugs, including indications, adverse effects and treatments applied. Chi-square and Fisher's exact tests were performed to analyze associations of data described by absolute and relative frequencies with professionals' time since graduation. All analyses were performed using the R software, with a 5% significance level. Results: Those who revealed to have graduated for more than five years with the highest academic degree were those who demonstrated maximum knowledge of antiresorptive drugs or revealed that, somehow, they had information about them (p<0.05). Conclusion: Dental surgeons in Brazil who have more than five years since graduation have more scientific information about antiresorptive drugs and pharmacological procedures, which can positively contribute to the prevention of osteonecrosis of the jaws and treatment of drug sequelae that may occur.

AIMS: To study the clinical and laboratory findings between patients with primary Antiphospholipid Syndrome (pAPS) with and without LR. BACKGROUND: Livedo Reticularis (LR) is a common manifestation of Antiphospholipid Syndrome (APS). Although no previous study evaluated patients with and without LR. METHODS: A transversal study including 66 pAPS patients was performed. Demographical, anthropometric, medication use, antiphospholipid antibodies profile data were evaluated, and LR's clinical and laboratory features. Patients were subdivided into one of two groups: pAPS with LR and pAPS without LR. RESULTS: Both groups were alike concerning demographics and anthropometrics. Interestingly, the frequency of stroke (28.5 vs. 7.5%, p=0.04), as well as of Sneddon's syndrome (100 vs. 30.0%, p<0.0001), were higher in pAPS with LR than the other group. Conversely, patients in the pAPS without LR group had more thyroidopathy than those in the pAPS with LR group (80% vs. 50% %, p=0.03). CONCLUSION: Patients with pAPS and LR have more stroke and seem to be protected from thyroidopathy. Careful follow-up of these patients is therefore advised.

BACKGROUND: Oral candidiasis is an opportunistic disease caused by fungi of the Candida genus. The occurrence of Candida spp. resistance to the commercial antifungal drugs points to the search for alternative treatments. Propolis has been successfully used in the treatment of infectious diseases for centuries. It has been proposed that an ultrasound pretreatment in the propolis extraction protocol can enhance the concentrations of molecules with antimicrobial activities in the final extract. Thus, this study aimed to compare the antifungal activity against oral Candida spp. isolates of green and red propolis extracts submitted or not to an ultrasound pretreatment before the extraction procedure. METHODS: Candida spp. were isolated from denture stomatitis lesions and identified by sequencing. Oral Candida spp. isolates and reference strains were submitted to broth microdilution assays using commercial antifungals and Brazilian green and red propolis extracts submitted or not to an ultrasound pretreatment. Minimal Inhibitory Concentrations (MIC) and Minimal Fungicide Concentrations (MFC) were determined and biofilm formation interference was evaluated for resistant isolates. RESULTS: C. albicans, Candida tropicalis and Candida dubliniensis were isolated from denture stomatitis lesions. Growth inhibition was observed in all Candida isolates incubated with all green and red propolis extracts. At lower doses, red propolis extracts presented significant antifungal activity. The ultrasound pretreatment did not promote an increase in the antifungal activity of green or red propolis. Three isolates, which were highly resistant to fluconazole and itraconazole, were susceptible to low doses of red propolis extracts. These same three specimens had their biofilm formation inhibted by red propolis ethanolic extract. CONCLUSIONS: Thus, red propolis can be faced as a promising natural product to be used in the auxiliary antifungal therapy of denture stomatitis.

PURPOSE: To describe the performance of elderly individuals in Pitch Pattern Sequence (PPS) and Duration Pattern Sequence (DPS) tests and research related factors. METHODS: An observational, cross-sectional study conducted with elderly people aged 60 to 79 years. The participants underwent cognitive screening tests, interviews containing socio-demographic data and general health, as well as audiologic evaluation and temporal auditory processing (PPS and DPS) evaluation tests. A descriptive analysis of the association between the performance in temporal processing and the variables gender, age, level of education and audiometric alterations was conducted through multiple linear regression. RESULTS: 86 elderly people participated in the study, most of them female, with ages between 60 and 69. Male participants performed better in both tests as well as the participants with higher education, whereas no difference in performance was observed across the different age ranges. In the DPS, the participants with auditory alteration performed worse in relation to the ones with average frequencies of 0.5 to 4 kHz. CONCLUSION: The male gender and higher level of education were associated with better results in the temporal ordering tests, whereas auditory alteration was associated with worse performance only in the pitch pattern sequence test.


OBJETIVO: Investigar os fatores associados e descrever o desempenho no teste padrão de frequência e teste padrão de duração em idosos. MÉTODO: Estudo observacional, seccional, conduzido com idosos de 60 a 79 anos. Os participantes realizaram teste de rastreio cognitivo, entrevista contendo dados sociodemográficos e de saúde geral, avaliação audiológica e testes de avaliação do processamento auditivo temporal (padrão de frequência e padrão de duração). Foi conduzida análise descritiva da associação entre o desempenho nos testes de processamento temporal e as variáveis: sexo, idade, nível de escolaridade e alterações audiométricas, por meio da regressão linear múltipla. RESULTADOS: Participaram do estudo 86 idosos, sendo a maioria do sexo feminino, com idade entre 60 e 69 anos. O desempenho para ambos os testes foi melhor nos homens e nos idosos com maior nível de escolaridade e similar nas diferentes faixas etárias. No teste padrão de frequência, idosos com alteração auditiva apresentaram pior desempenho do que aqueles com média nas frequências de 0.5 a 4 kHz normal. CONCLUSÃO: O sexo masculino e maior nível de escolaridade estão associados ao melhor desempenho nos testes de ordenação temporal, enquanto a alteração audiométrica apresenta associação com o pior desempenho apenas no teste padrão de frequência.

Introdução: devido à grande prevalência do transtorno do espectro autista (TEA) e da esquizofrenia e o caráter incapacitante dessas patologias, fez-se necessário o desenvolvimento de tratamentos cada vez mais eficientes e com menos efeitos adversos. Sendo a microbiota um regulador da inflamação e tendo esta um papel fundamental na gênese dos transtornos psiquiátricos, o uso de probióticos pode ser uma alternativa segura e com baixo custo para o tratamento complementar das doenças psiquiátricas. Objetivo: realizar uma revisão narrativa sobre a eficácia do uso de probióticos na redução de sintomas clínicos e gastrointestinais, assim como da segurança no tratamento do TEA e da esquizofrenia. Metodologia: revisão dos estudos em humanos que avaliaram os efeitos terapêuticos dos probióticos no TEA e esquizofrenia. Resultados: foram selecionados 13 estudos, sendo 8 sobre TEA e 5 sobre esquizofrenia, com total de 485 pacientes. As evidências apontam melhora dos sintomas gastrintestinais dos pacientes com TEA e esquizofrenia; entretanto, sem mudanças conclusivas dos sintomas psiquiátricos na esquizofrenia, associado a potencial benefício no TEA. Conclusão: essa revisão sugere que o uso de probióticos tem um potencial benefício nos sintomas comportamentais no TEA, associado à melhora dos sintomas gastrointestinais na esquizofrenia e TEA; entretanto, não se observou mudanças nos sintomas psiquiátricos da esquizofrenia.


Introduction: due to the high prevalence of autism spectrum disorder (ASD) and schizophrenia and the disabling nature of these pathologies, it has become necessary to develop increasingly efficient treatments with less adverse effects. As the microbiota is a regulator of inflammation and having a fundamental role in the genesis of psychiatric disorders, the use of probiotics may be a safe and low-cost alternative for the complementary treatment of psychiatric diseases. Objective: to perform a narrative review on the fficacy of probiotics use in reducing clinical and gastrointestinal symptoms, and adverse effects of the use of probiotics in the treatment of ASD and schizophrenia. Methodology: review of studies in humans that evaluated the therapeutic effects of probiotics on ASD and schizophrenia. Results: 13 studies were selected, 8 on TEA and 5 on schizophrenia, with a total of 485 patients. Evidence points to improvement in gastrointestinal symptoms in patients with ASD and schizophrenia; however, there were no conclusive changes in psychiatric symptoms in schizophrenia, associated with a potential benefit in ASD. Conclusion: This review suggests that the use of probiotics has a potential benefit in behavioral symptoms in ASD, associated with the improvement of gastrointestinal symptoms in schizophrenia and ASD; however, there were no changes in the psychiatric symptoms of schizophrenia.

Introdução: a Síndrome Antifosfolípide é caracterizada por eventos trombóticos e perdas gestacionais de repetição e é considerada a trombofilia adquirida mais comum. Quando não está associada a alguma doença do tecido conectivo é dita primária e seu tratamento é baseado em anticoagulação por longo período com warfarin. Isso requer controle rigoroso do tempo de protrombina realizada pela monitoração dos valores de INR para que se evite em um extremo o risco de sangramento e em outro o risco de trombose. Objetivo: realizar uma revisão daa literatura sobre polimorfismos genéticos da citocromo P450 na síndrome antifosfolipide. Metodologia: revisão narrativa da literatura. Resultados: embora investigações tenham identificado a influência de vários genes na resposta ao warfarin, a maioria das evidências sugere um papel mais importante para o polimorfismo de dois genes: o gene do citocromo P450(CYP)2C9 (CYP2C9) e o gene do complexo redutase epóxido vitamina K 1 (VKORC1). De fato, o warfarin é administrado como uma mistura racêmica de S ­ e R-warfarin e estes enantiômeros são extensivamente metabolizados no fígado por diferentes enzimas do citocromo P450(CYP), com a CYP2C9 servindo como a principal enzima no metabolismo da S-warfarin. Nesse sentido, a farmacogenética da terapia com warfarin é relevante para melhorar a segurança e a efetividade dessa terapia. Polimorfismos estruturais no gene CYP2C9 criam variantes alélicos que codificam enzimas com diferentes atividades catalíticas. As freqüências alélicas destes variantes diferem entre diferentes grupos étnicos, sem estudos no Brasil em pacientes com SAF. Conclusão: o conhecimento da presença de polimorfismos genéticos da citocromo P450 em usuários de warfarin é de fundamental importância. Desde que sangramentos ou alvos subterapeuticos podem advir da presença dessas alterações genéticas.


Introduction: antiphospholipid Syndrome is characterized by thrombotic events and repeated pregnancy losses and is considered the most common acquired thrombophilia. When it is not associated with any connective tissue disease, it is said to be primary and its treatment is based on long-term anticoagulation with warfarin. This requires strict control of the prothrombin time performed by monitoring the INR values to avoid the risk of bleeding at one extreme and the risk of thrombosis at the other. Objective: To perform a review of the literature on genetic polymorphisms of cytochrome P450 in antiphospholipid syndrome. Methods: narrative literature review Results: although investigations have identified the influence of several genes on the response to warfarin, most evidence suggests a more important role for the polymorphism of two genes: the cytochrome P450 (CYP) 2C9 (CYP2C9) gene and the reductase complex gene vitamin K 1 epoxide (VKORC1). In fact, warfarin is administered as a racemic mixture of S ­ and R-warfarin and these enantiomers are extensively metabolized in the liver by different enzymes of cytochrome P450 (CYP), with CYP2C9 serving as the main enzyme in the metabolism of S-warfarin . In this sense, the pharmacogenetics of warfarin therapy is relevant to improve the safety and effectiveness of this therapy. Structural polymorphisms in the CYP2C9 gene create allelic variants that encode enzymes with different catalytic activities. The allele frequencies of these variants differ between different ethnic groups, with no studies in Brazil in patients with APS. Conclusion: knowledge of the presence of genetic polymorphisms of cytochrome P450 in users of warfarin is of fundamental importance. Since bleeding or subtherapeutic targets may result from the presence of these genetic changes.

Introdução: a dor no ombro leva a limitações e incapacidades entre adultos e idosos, merecendo atenção do fisioterapeuta. Objetivo: Avaliar o efeito imediato da técnica de Mulligan em pacientes com dor no ombro. Metodologia: trata-se de resultados preliminares de um ensaio clínico realizado entre julho de 2018 a julho de 2019. Pacientes com dor no ombro (n=44), de ambos os sexos, foram randomizados e submetidos a dois protocolos de tratamento: exercícios terapêuticos (n=22) e MWM de Mulligan (n=22), atendidos na Clínica Escola de Fisioterapia da UFBA. Todos os participantes responderam a um questionário clínico e sociodemográfico e realizaram avaliação da dor pela escala visual analógica (EVA) e algometria. Os pacientes foram tratados por dois fisioterapeutas experientes na área e, depois, foram reavaliados imediatamente. Resultados: mulheres predominaram, a média de idade foi de 56 anos no grupo Mulligan e 57 no grupo exercícios terapêuticos. Síndrome do manguito rotador predominou: 19 (86,4%) no grupo Mulligan e 15 (68%) no grupo exercícios. O ombro mais acometido foi o direito no Grupo Mulligan, (17 (77,3%); já no grupo exercícios, foi o esquerdo com 12 (55%). A cor negra foi a mais autorrelatada pelos pacientes, e poucos tinham o ensino superior completo. No grupo Mulligan, as ocupações mais informadas foram dona de casa, seguida de aposentados e autônomos. No grupo exercícios, não houve dona de casa, e sim mais aposentados e autônomos. Os dois grupos mostraram ser mais sedentários. Não houve diferença da dor pela EVA entre os grupos, antes e após o tratamento (p=0.79, p=0.56, respectivamente). Já a intensidade da dor mensurada pela algometria, tanto antes do tratamento (p=0.008) quanto depois do tratamento (p=0.04), foi diferente entre os grupos, com predomínio de melhora no grupo Exercícios. Conclusão: a MWM não foi mais eficaz que os exercícios terapêuticos na redução da dor do ombro, sendo que o exercício apresentou mais eficácia quando medido através da EVA e sem melhora significativa ao ser mensurada pela algometria após aplicação de uma única sessão de tratamento.


Introduction: shoulder pain leads to limitations and disabilities among adults and the elderly deserving attention from the physiotherapist. Objective: to evaluate the immediate effect of the Mulligan technique in patients with shoulder pain. Methodology: these are preliminary results of a clinical trial conducted from July 2018 to July 2019. Patients (n=44) with shoulder pain of both sexes were randomized and submitted to two treatment protocols: therapeutic exercises (n = 22) and Mulligan's MWM (n = 22) attended at the UFBA School of Physiotherapy Clinic. All participants answered a clinical and sociodemographic questionnaire and underwent pain assessment by visual analog scale (VAS) and algometry. The patients were treated by two experienced physiotherapists and then reassessed immediately. Results: women predominated, the average age was 56 years in the Mulligan group and 57 in the therapeutic exercises group. Rotator cuff syndrome predominated, 19 (86.4%) in the Mulligan group and 15 (68%) in the exercise group. The most affected shoulder was the right shoulder in the Mulligan Group (17 (77.3%), while in the exercise group was the left shoulder with 12 (55%).The black color was the most self-reported by the patients and few patients had higher education. In the Mulligan group the most informed occupations were housewife followed by retirees and self-employed. In the exercise group we had no housewife but more retired and self-employed. The two groups were more sedentary. There was no difference in VAS pain between the groups before and after treatment (p = 0.79, p = 0.56, respectively), while pain intensity measured by algometry both before treatment (p = 0.008) and after treatment (p = 0.04) was different between Conclusion: Mulligan's MWM was not more effective than exercise in reducing shoulder pain, and exercise was more effective when measured by VAS and without significant improvement when measured by algometry after applying a single treatment session.

Não há consenso na literatura quanto à associação entre postura craniocervical (PCC), disfunção temporomandibular (DTM) e oclusão dentária. Ainda assim, admite-se que a articulação temporomandibular está intimamente envolvida com a biomecânica cervical e cintura escapular. Objetivo: traçar o perfil das associações entre PCC, DTM e oclusão dentária. Metodologia: estudo observacional em corte transversal realizado em universitários, de ambos os sexos, com idade entre 18 e 40 anos. Os voluntários foram avaliados pelo RDC/TMD, biofotogrametria, sendo esta realizada através do Software de Avaliação Postural (SAPO) pelos ângulos A1(tragushorizontal), A2 (tragus-C7-horizontal), A3 (tragus ­ acrômio-vertical) e distância tóraco-cervical, além da análise quanto ao tipo de oclusão dentária. O tratamento dos dados foi feito a partir de tendência central, razão de prevalência e análise de correspondência múltipla assimétrica. Resultados: foram avaliados 88 voluntários com mediana da idade de 22 anos, sendo 27 do sexo masculino e 62 do feminino. A amostra foi dividida em quatro grupos, de acordo com a presença de DTM, padrão de dor à palpação, cefaléia, PCC e oclusão. As maiores associações observadas foram: DTM tipo II com a distoclusão tipo dois, DTM tipo I com flexão de cabeça e retificação da coluna cervical; mesioclusão com alinhamento da coluna cervical e indivíduos sem dor com anteriorização da cabeça e distoclusão tipo um. Conclusão: este estudo demonstrou que a DTM não interfere na postura da cabeça, mas que a má oclusão pode alterar a postura craniocervical nos planos sagital e frontal além de aumentar a lordose cervical.


There is no consensus in the literature about the association between craniocervical posture (CCP), temporomandibular dysfunction (TMD) and dental occlusion, although it is admitted that the temporomandibular joint is closely involved with the cervical and shoulder girdle biomechanics. Objective: to identify the profile about associations between CCP, TMJ and dental occlusion. Methodology: observational cross-sectional study, conducted on both sexes college student aged 18 to 40 years. The subjects were evaluated by RDC/TMD, biophotogrammetry, using Postural Assessment Software (SAPO), according to A1 (tragus-horizontal), A2 (tragus-C7- horizontal), A3 (tragus-acromion-vertical) and cervical thoracic distance, besides the analysis of the type of dental occlusion. Data analysis was performed by measures of central tendency, prevalence ratio and asymmetric multiple correspondence. Results: a total of 88 volunteers with a median age of 22 years were evaluated, 27 male and 62 female. The profiles was divided into four groups according to the presence of TMD, pain on palpation, headache, CCP and occlusion. The associations observed were: type II TMD with type two distocclusion, type I TMD with head flexion and cervical spine rectification; mesiocclusion with cervical spine alignment and pain-free individuals with anteriorization of the head and distocclusion type one. Conclusão: this study demonstrated that TMD does not interfere with the head position, but that malocclusion may alter craniocervical posture in the sagittal and frontal planes and increase cervical lordosis.

Objective: This study investigated the effect of endogenous erosion on the microhardness of dentine and a nanofilled composite resin. Procedures for preventing erosion were also studied. Materials and Methods: 90 bovine dentine specimens were divided into three groups in accordance with the method for preventing: negative control, topical application of fluoride and resin-modified glass ionomer varnish. 120 composite resin specimens were distributed into four groups, which also included a resin sealant, among the preventive procedures. Specimens were then randomly divided into three sub-groups according to the exposure to simulate gastric acid solution and subsequent remineralization: negative control, 9 and 18 cycles. Surface analysis was carried out by measuring the Knoop hardness. The data obtained were statistically analyzed using 2-way ANOVA and Tukey test. Result: The mean hardness of dentine and of the composite specimens resin exhibited lower hardness after 18 cycles. However, the resin-modified glass ionomer varnish resulted in greater values compared to the other preventive procedures. Conclusion: A resin-modified glass ionomer varnish seems to be a promising method for minimizing the damage caused by endogenous acid, but its protection can be reduced depending on the intensity of the erosive challenge.


Objetivo: Este estudio investigó el efecto de la erosión endógena sobre la microdureza de la dentina y una resina compuesta de nanorrelleno. También se estudiaron los procedimientos para prevenir la erosión. Materiales and Métodos:90 muestras de dentina bovina se dividieron en tres grupos de acuerdo con el método para prevenir: control negativo, aplicación tópica de fluoruro y barniz de ionómero de vidrio modificado con resina. Se distribuyeron 120 muestras de resina compuesta en cuatro grupos, que también incluían un sellador de resina, entre los procedimientos preventivos. Las muestras se dividieron al azar en tres subgrupos de acuerdo con la exposición para simular la solución de ácido gástrico y la remineralización posterior: control negativo, 9 y 18 ciclos. El análisis de la superficie se realizó midiendo la dureza Knoop. Los datos obtenidos se analizaron estadísticamente mediante ANOVA de 2 vías y prueba de Tukey. Resultados: La dureza media de la dentina y de la resina de muestras compuestas exhibió una dureza más baja después de 18 ciclos. Sin embargo, el barniz de ionómero de vidrio modificado con resina resultó en valores mayores en comparación con los otros procedimientos preventivos. Conclusión: Un barniz de ionómero de vidrio modificado con resina parece ser un método prometedor para minimizar el daño causado por el ácido endógeno, pero su protección puede reducirse dependiendo de la intensidad del desafío erosivo.

RESUMO Objetivo Investigar a influência dos domínios cognitivos no desempenho do teste padrão de frequência e teste padrão de duração em idosos. Métodos estudo seccional, desenvolvido em indivíduos com idade entre 60 e 79 anos. Realizou-se entrevista, avaliação cognitiva por meio do Montreal Cognitive Assessment, avaliação audiológica periférica (audiometria tonal e vocal) e central (teste padrão de frequência e teste padrão de duração). Resultados Participaram do estudo 58 mulheres com média de idade de 66 anos e 2 meses e 28 homens, com média de idade de 68 anos e 3 meses. Verificou-se que as habilidades visuoespacial, de atenção, concentração e memória de trabalho apresentaram correlação com os testes temporais no sexo feminino e que a habilidade de linguagem apresentou correlação com o teste padrão de frequência. Já entre os homens, houve tendência à significância quanto à capacidade visuoespacial. Ademais, as mulheres apresentaram melhor desempenho na habilidade de memória. Conclusão Os aspectos cognitivos podem influenciar nos testes de ordenação temporal em indivíduos idosos, sobretudo do sexo feminino.


ABSTRACT Purpose To investigate the influence of cognitive domains on the performance of the pitch pattern sequence and duration pattern sequence tests among elderly people. Methods Cross-study realized among individuals aged between 60 to 79 years. We performed an interview, cognitive evaluation through Montreal Cognitive Assessment, peripheral audiologic evaluation (tonal and vocal audiometry) and central (pitch pattern sequence and duration pattern sequence). Results The study involved 58 women with average age of 66.2, and 28 men at an age average of 68.3. On the one hand, we observed that the visual-spatial abilities, attention, concentration and working memory present correlation with temporal tests in females and that language ability correlates with the standard frequency test. Among men, there was a tendency to significance in terms of visual-spatial abilities. Furthermore, women had better performance in memory ability. Conclusion Cognitive aspects may influence the temporal ordering tests among elderly individuals, especially females.

ABSTRACT Purpose: to investigate the possible association between the severity of the temporomandibular disorder, cervical pain, and mandibular function impairment. Methods: is a cross-sectional, descriptive study, conducted with 32 individuals with temporomandibular disorder, categorized by degree of severity, according to the Fonseca Index. Using the diagnosis criteria for temporomandibular disorder, the likely etiological factors for the disorder were established, as well as the intensity of the functional disability, resulting from cervical pain and of the mandibular impairment. The data obtained were statistically treated, adopting the significance level of 5%. Results: the mean age was 33.8 years, 90.6% being females. As for the degree of disorder, 56.3% presented severe TMD, followed by 28.1% showing a moderate one. The myogenic etiology was present in 93.7% of the patients. Cervical pain was present in 90.6% of them, of which, 59.4% presented a mild disability, and 25%, a moderate one. Considering the mandibular function, 46.9% of the patients presented a low, 40.6%, a moderate, and 12.5%, a severe impairment. There was a statistically significant association between cervical pain and mandibular function (p = 0.011). However, although there was an increase in cervical disability and in mandibular impairment as the severity of the TMD also increased, these associations were not statistically significant (p = 0.178 and p = 0.102, respectively). Conclusion: it can be stated that there is a higher prevalence of severe TMD and of myogenic origin, and that cervical pain influences, directly, the mandibular function, which is not necessarily related to the severity of the temporomandibular alteration. Likewise, such severity does not present a relationship to mandibular function impairment either.


RESUMO Objetivo: investigar possível associação entre severidade da disfunção temporomandibular, cervicalgia e limitação funcional mandibular. Métodos: estudo seccional descritivo, em 32 indivíduos com disfunção temporomandibular, categorizados de acordo com o grau de severidade segundo o Índice de Fonseca. Utilizando-se o Critérios Diagnósticos de Pesquisa em Disfunção Temporomandibular, estabeleceram-se prováveis fatores etiológicos da disfunção e a intensidade de incapacidade funcional cervical e mandibular, avaliados pelo Índice de Incapacidade Relacionada à Dor no Pescoço e Questionário de Limitação Funcional Mandibular respectivamente. Os dados obtidos foram tratados estatisticamente, adotando-se nível de significância de 5%. Resultados: a média de idade foi de 33,8 anos, sendo 90,6% pertencentes ao gênero feminino. Quanto à severidade da disfunção, 56,3% possuíam o tipo severo, sendo moderada em 28,1%. Em 93,7%, verificou-se etiologia miogênica. A cervicalgia esteve presente em 90,6% dos pacientes, dos quais 59,4% apresentaram incapacidade leve e 25% moderada. Quanto à função mandibular, houve baixa limitação em 46,9%, moderada em 40,6%, tendo sido severa em 12,5%. Houve associação estatisticamente significante entre a cervicalgia e a função mandibular (p=0,011). Contudo, embora tenha havido aumento na incapacidade cervical e na limitação mandibular com o aumento da severidade da disfunção temporomandibular, estas associações não foram estatisticamente significantes (p=0,178 e p=0,102, respectivamente). Conclusão: é possível afirmar que há maior prevalência de disfunção temporomandibular de intensidade severa e de origem miogênica, e que a cervicalgia influencia diretamente a função mandibular, não estando necessariamente relacionada à severidade da disfunção temporomandibular. Da mesma forma, esta última também não apresenta relação direta com a limitação da função mandibular.

Abstract Objective: To determine the relationship between tobacco and alcohol consumption and staging of head and neck cancer in patients treated at a referral hospital for cancer treatment. Material and Methods: This is a cross-sectional study based on medical records of patients with head and neck cancer treated between 2008 and 2015. The following variables were considered: gender, age, marital status, educational level, place of residence, profession, affected site, clinical staging of neoplasms, histopathological diagnosis, tobacco and alcohol consumption. Results: Of the 154 medical records analyzed, there was a predominance of male patients (72.7%), single (50%), with low education (88.1%), rural workers (38.3%), residents in the state countryside (70.2%) and aged 20-94 years. T3 and T4 size tumors prevailed. A significant correlation was found between alcohol use and tumor size (p=0.03) and presence of nodules (p<0.001), as well as between alcohol use and smoking with tumor size (p=0.04) and presence of nodules (p=0.019). No correlations were observed between tobacco use and injury staging. Conclusion: There was strong significance between tumor size and presence of nodules, both with the use of alcohol alone and with tobacco, thus suggesting the existence of important carcinogenic action of this substance.

The dentin exposed to erosive challenges is often superficially protected to prevent progression of the lesion. This study investigated the marginal and internal adaptation of composite resin restorations made on surfaces that had first been treated with different methods for controlling erosion. Cavities with margins in dentin were prepared in bovine incisors (n=360) and were divided into three groups according to the method for controlling the erosive challenge: negative control, topical application of fluoride and resin-modified glass ionomer varnish. The specimens were then randomly divided into three sub-groups according to the exposure to simulated gastric acid solution (DES) (5% HCl, pH=2.2) and subsequent remineralization (RE): negative control, 9 and 18 cycles of DES-RE. Finally, teeth were divided into four groups, depending on the bonding agent used for composite resin restoration (n=10): conventional etch-and-rinse adhesive system (2 and 3 steps) and self-etching (1 and 2 steps). Front and internal images of the interface tooth/restoration were recorded in stereoscopic microscope (15x) to quantify the percentage of adhesive failures. Despite the promising results of the resin-modified glass ionomer varnish after 9 cycles; no protective material prevented increased internal defects after 18 erosive cycles. More continuous internal margins were noted with etch-andrinse acid systems after more intense erosion. The maintenance of internal margins in eroded substrates was positively influenced by the resinmodified glass ionomer varnish and, under the most aggressive challenge, by the use of etch-and-rinse adhesives systems. A dentina exposta a desafios erosivos é muitas vezes superficialmente protegida para prevenir a progressão da lesão. Este estudo investigou a adaptação marginal e interna de restaurações de resina composta realizadas em superfícies que foram previamente tratadas com diferentes métodos para controlar a erosão. Cavidades com margens em dentina foram preparadas em dentes incisivos bovinos (n=360) e divididas em três grupos de acordo com o método de controle do desafio erosivo: controle negativo, aplicação tópica de flúor e verniz de ionômero de vidro modificado por resina. Os espécimes foram então divididos aleatoriamente em três subgrupos de acordo com a exposição à solução de ácido gástrico simulada (DES) (HCl a 5%, pH = 2,2) e posterior remineralização (RE): controle negativo, 9 e 18 ciclos de DES-RE. Por fim, os dentes foram divididos em quatro grupos, de acordo com o agente de união utilizado para a restauração da resina composta (n=10): sistema adesivo convencional (2 e 3 passos) e autocondicionante (1 e 2 passos). Imagens frontais e internas da interface dente/restauração foram registradas em microscópio estereoscópico (15x) para quantificar a porcentagem de falhas adesivas. Apesar dos resultados promissores do verniz de ionômero de vidro modificado por resina após 9 ciclos; nenhum material de proteção impediu o aumento de defeitos internos após 18 ciclos erosivos. Margens internas mais contínuas foram observadas com sistemas adesivos convencionais após erosão mais intensa. A manutenção das margens internas em substratos erodidos foi influenciada positivamente pelo verniz de ionômero de vidro modificado por resina e, sob o desafio mais agressivo, pelo uso de sistemas de adesivos convencionais. PURPOSE: Describe hearing complaints and alterations in individuals with systemic sclerosis (SS) and to verify the development of audiological manifestations. METHODS: This is a cross-sectional study with a prospective phase, conducted in the period from 2012 to 2015, with patients with medical diagnosis of SS. Sociodemographic data, year of disease onset, year of diagnosis and disease subtype were collected. Later, audiological anamnesis was performed to identify complaints and symptoms and to investigate the performance of audiometry before the study and, after that, a basic audiological evaluation was conducted. RESULTS: Fifty individuals participated in the study. Dizziness and tinnitus were the most frequent symptoms. Hearing loss was identified in 23 (46%) individuals; most of them were of sensorineural type, of variable degrees and configurations. The analysis of hearing thresholds obtained in the audiological evaluation performed in 2012 and, later, in 2015, indicated onset or progression of hearing loss, with aggravation of 10dB in most frequencies evaluated, being more expressive in acute frequencies. CONCLUSION: High rate of hearing complaints and alterations in individuals with SS and onset and/or progression of hearing loss in those who underwent serial audiological evaluation were observed.

A análise dos limares auditivos obtidos na avaliação audiológica realizada em 2012 e, posteriormente, em 2015 indicou desencadeamento ou progressão da perda auditiva, com piora de 10dB na maioria das frequências avaliadas, sendo mais expressiva nas frequências agudas. CONCLUSÃO: Elevada frequência de queixas e alterações auditivas em indivíduos com ES e desencadeamento e/ou progressão da perda auditiva naqueles que realizaram avaliação audiológica sequencial.

Resumo Introdução O preparo do canal radicular, em especial o terço apical, é fundamental para o sucesso da terapia endodôntica, pois abriga o forame radicular, local que comunica o endodonto com os tecidos perirradiculares. Objetivo A presente pesquisa avaliou a contaminação do terço apical após a instrumentação de canais radiculares em três comprimentos distintos, através de dois tipos de instrumentação automatizada (rotatória contínua ou reciprocante) e uso de duas substâncias irrigadoras. Material e método Foram selecionados 66 pré-molares superiores birradiculares, os quais tiveram suas coroas removidas e raízes separadas, totalizando 132 corpos de prova (CPs). Na primeira fase, foram estabelecidos por sorteio seis grupos experimentais divididos de acordo com o comprimento de instrumentação (aquém, além e no forame radicular) e o tipo de instrumentação (rotatória contínua ou reciprocante), tendo como irrigante o hipoclorito de sódio. Na segunda fase, foi repetido o mesmo protocolo, utilizando-se como irrigante o cloreto de sódio. Para ratificar os resultados encontrados, foram criados dois grupos controles: positivo e negativo, cada um com seis CP. Resultado Não foram encontradas diferenças entre os grupos com instrumentação rotatória contínua ou reciprocante, nos seus diversos comprimentos de instrumentação, respeitando-se o mesmo irrigante. Os grupos irrigados com hipoclorito não apresentaram crescimento bacteriano, enquanto os irrigados com cloreto de sódio permaneceram contaminados após a instrumentação. Conclusão A ação mecânica dos instrumentos endodônticos sem o uso de substância irrigadora bactericida não promove a descontaminação apical do canal radicular e a escolha do tipo da cinemática dos instrumentos endodônticos (rotatória contínua ou reciprocante) não influencia significativamente a redução de bactérias no interior dos canais radiculares, bem como instrumentações que variam de um milímetro aquém até um milímetro além do forame também não apresentaram diferenças na redução de bactérias nesta pesquisa.


Abstract Introduction The preparation of the root canal is fundamental for the success of endodontic therapy, especially the apical third, since it contains the root foramen, where the endodontium and the periradicular tissues communicate. Objective The present study evaluated the contamination of the apical third after the root canal instrumentation in three different lengths, through two types of automated instrumentation (continuous or reciprocating rotating) and two irrigating substances used. Material and method Sixty-six biradicular maxillary premolars were selected, which had their crowns removed and roots separated, totaling 132 specimens. In the first phase, six experimental groups were divided according to the length of instrumentation (before, beyond and in the radicular foramina) and the type of instrumentation (continuous or reciprocating rotatory) with sodium hypochlorite as irrigator. In the second phase the same protocol was repeated, using sodium chloride as the irrigant. In order to confirm the results, two control groups were created: positive and negative, each with six specimens. Result For the same irrigant, no differences were found between the groups with continuous or reciprocating rotational instrumentation in their different working lengths. The groups irrigated with hypochlorite showed no bacterial growth, whereas those irrigated with sodium chloride remained contaminated after instrumentation. Conclusion Mechanical action of endodontic instruments, without bactericidal irrigating substance, does not promote the apical root canal disinfection and the choice of kinematic type of endodontic instruments (continuous or reciprocating rotatory) does not significantly influence the reduction of bacteria within the root canals as well instrumentations ranging from one millimeter before to one millimeter beyond the foramen also did not present differences in the reduction of bacteria in this research.

Aim: The study investigated the effect of using surface protection agents in the adaptation of external and internal margins of restorations subjected to simulated erosion. Methods: Cavities with margins in dentin were prepared in bovine incisors (n=120). Adhesive restorations were placed using a three-step etch&rinse adhesive system and nanofilled composite resin. The specimens were divided into four groups, according to the surface protection: negative control, topical application of fluoride (TAF), resin sealant and resin-modified glass ionomer varnish (RMGI varnish). Afterwards, they were divided into three sub-groups, according to the exposure to a simulated solution of gastric acid (DES) (5% HCl, pH=2,2) and subsequent remineralization (RE): negative control, 9 and 18 cycles of DES-RE. The evaluation of the tooth-restoration interface was performed on the internal and frontal images with the aid of a stereoscopic microscope (15x), and the percentage of continuous margins without adhesive failures was quantified. Results: In the external margins, only those groups with surface protection using sealants (resin and glass-ionomer) did not exhibit a significant decrease in the percentage of continuous margins after the erosive challenges. After 18 cycles of DES-RE, the use of resin-modified glass ionomer varnish resulted in the highest percentage of continuous margins. Conclusion: It was concluded that physically covering the surfaces with a sealing agent preserved the marginal adaptation of composite resin restorations exposed to endogenous erosive challenges
