RESUMEN

Mandibular third molar (M3) removal is a routine surgical procedure that can generate a variety of complications, one of the rarest of which is mandibular fracture. The aim of this paper is to report a rare case of bilateral fracture of the jaw during the extraction of mandibular third molars. A 42-year-old woman complained of mandibular pain and malocclusion after the removal of the M3s. The physical examination revealed bone crepitation mandibular bilaterally. Computed tomography confirmed bilateral fracture of the jaw. The patient was submitted to the reduction and fixation of the fractures under general anesthesia, followed by the improvement in the postoperative period. Bilateral mandibular fracture resulting from M3 extractions is a rare occurrence that may be caused by a lack of surgical skill and a flawed diagnosis. The proper, immediate correction could lead to the interruption of the procedure at the time of the first fracture, thereby avoiding the contralateral fracture. Introdução: A osteotomia Le Fort I possibilita a correção de deformidades dentofaciais que envolvem o terço médio da face. Para sua fixação, convencionou-se o emprego de quatro mini-placas nos pilares zigomático-maxilar e nasomaxilar. Propôs-se então, a dispensa da fixação do segmento posterior, surgindo questionamentos relacionados à capacidade biomecânica do sistema. Objetivos: Comparar o estresse biomecânico gerado em três meios distintos de fixação da osteotomia Le Fort I frente ao movimento de avanço sagital linear maxilar de 7mm. Metodologia: Trata-se de uma pesquisa experimental laboratorial, utilizando-se da análise de elementos finitos como ferramenta analítica, a fim de constatar qual das técnicas sofrerá maior estresse biomecânico. Resultados: Constatou-se que o estresse biomecânico gerado é maior quando aplicado em 4 pontos do que quando aplicado em apenas 2 pontos. Conclusão: Os resultados obtidos fornecem informações aos cirurgiões sobre a real necessidade do uso de fixação adicional de acordo com o método de fixação planejado. No entanto, deve ser interpretado de forma cautelosa, considerando-se as limitações deste estudo. Sendo assim, uma análise incipiente, que tem como intuito o fornecimento de evidência científica de grande significância.


Introducción: La osteotomía Le Fort I permite la corrección de deformidades dentofaciales que involucran el tercio medio de la cara. Para su fijación se acordó utilizar cuatro miniplacas en los pilares cigomaticomaxilar y nasomaxilar. Entonces se propuso prescindir de la fijación del segmento posterior, planteando interrogantes relacionados con la capacidad biomecánica del sistema. Objetivos: Comparar el estrés biomecánico generado en tres medios diferentes de fijación de la osteotomía Le Fort I frente a un movimiento de avance sagital lineal maxilar de 7mm. Metodología: Se trata de una investigación experimental de laboratorio, utilizando como herramienta analítica el análisis de elementos finitos, con el fin de comprobar cuál de las técnicas sufrirá un mayor estrés biomecánico. Resultados: Se encontró que el estrés biomecánico generado es mayor cuando se aplica en 4 puntos que cuando se aplica solo en 2 puntos. Conclusión: Los resultados obtenidos brindan información a los cirujanos sobre la necesidad real de utilizar fijación adicional de acuerdo al método de fijación planificado. Sin embargo, debe interpretarse con cautela, considerando las limitaciones de este estudio. Por tanto, un análisis incipiente, que pretende aportar evidencias científicas de gran trascendencia.


Introduction: The Le Fort I osteotomy allows the correction of dentofacial deformities involving the middle third of the face. For its fixation, it was agreed to use four mini plates on the zygomaticomaxillary and nasomaxillary pillars. It was then proposed to dispense with the fixation of the posterior segment, raising questions related to the biomechanical capacity of the system. Objectives: To compare the biomechanical stress generated in three different means of fixation of the Le Fort I osteotomy against a 7mm maxillary linear sagittal advancement movement. Methodology: This is an experimental laboratory research, using finite element analysis as an analytical tool, in order to verify which of the techniques will suffer greater biomechanical stress. Results: It was found that the biomechanical stress generated is greater when applied to 4 points than when applied to only 2 points. Conclusion: The results obtained provide information to surgeons about the real need to use additional fixation according to the planned fixation method. However, it should be interpreted with caution, considering the limitations of this study. Therefore, an incipient analysis, which aims to provide scientific evidence of great significance.

RESUMEN

[RESUMO]. Objetivo. Delinear o panorama atual acerca dos agravos de saúde bucal da população quilombola no Brasil. Métodos. Uma busca foi realizada nas bases de dados Biblioteca Virtual em Saúde (BVS), PubMed, EMBASE, Scopus, Web of Science, Science Direct e SciELO, além de busca manual no Google Scholar, sem restrições de período. Foram incluídos artigos publicados em inglês, português ou espanhol descrevendo pesquisas realizadas no Brasil sobre saúde bucal em indivíduos que se autodeclararam quilombolas de comunidades reconhecidas pelo poder público. Analisou-se ainda a qualidade da evidência nos estudos incluídos. Resultados. Foram selecionados 14 estudos, todos com desenho transversal e classificados como nível IV de evidência científica (evidência proveniente de estudos descritivos ou estudos com abordagem qualita- tiva), ou seja, qualidade moderada-baixa. Seis estudos foram desenvolvidos na região Nordeste do Brasil e nove foram publicados entre 2016 e 2021. Sobre os agravos de saúde bucal, houve alta frequência de cárie dentária e altos índices de experiência da doença. O edentulismo e a doença periodontal também foram frequentes, afetando indivíduos jovens, adultos e idosos. Conclusão. Foi alta a prevalência de cárie dentária, edentulismo e doenças periodontais descrita na literatura sobre quilombolas no Brasil, demonstrando a necessidade de melhorar a assistência odontológica nesse grupo populacional.


[ABSTRACT]. Objective. To describe the current scenario of oral health in quilombola communities in Brazil. Methods. A search was performed in the Virtual Health Library (VHL), PubMed, EMBASE, Scopus, Web of Science, Science Direct, and SciELO, without date limits. Articles published in English, Portuguese, or Spanish were included if they described research performed in Brazil to investigate oral health in individuals self- declared as belonging to officially recognized quilombola communities. The quality of evidence of included studies was analyzed. Results. Fourteen studies were selected for inclusion, all with cross-sectional design and classified as level IV scientific evidence (evidence from descriptive studies or studies with a qualitative approach), i.e., moderate- low quality. Six were developed in the Northeast region of Brazil and nine were published between 2016 and 2021. Regarding oral health problems, a high frequency was detected for dental caries, with high indices of disease experience. Edentulism and periodontal disease were also frequent, affecting youth, adults, and older individuals. Conclusion. A high prevalence of dental caries, edentulism, and periodontal disease was observed, suppor- ting the need to improve dental care in this population group.


[RESUMEN]. Objetivo. Describir el panorama actual de los trastornos de salud bucodental de la población quilombola de Brasil. Métodos. Se realizó una búsqueda en las bases de datos de la Biblioteca Virtual en Salud (BVS), PubMed, EMBASE, Scopus, Web of Science, Science Direct y SciELO, además de una búsqueda manual en Google Scholar, sin restricciones del período. Se incluyeron artículos publicados en español, inglés o portugués descriptivos de investigaciones realizadas en Brasil sobre la salud bucodental de personas que se autode- nominan quilombolas pertenecientes a comunidades reconocidas por el poder público. También se analizó la calidad de la evidencia científica de los estudios incluidos. Resultados. Se seleccionaron 14 estudios, todos con un diseño transversal y clasificados en el nivel IV de evidencia científica (evidencia proveniente de estudios descriptivos o estudios con un enfoque cualitativo), es decir, de mediana y baja calidad. Se realizaron seis estudios en la región nordeste de Brasil y se publi- caron nueve en el período 2016-2021. En cuanto a los trastornos de salud bucodental, se observó una alta frecuencia de caries dental y altas tasas de prevalencia de esa enfermedad. El edentulismo y la enfermedad periodontal también fueron frecuentes y afectaron a jóvenes, adultos y personas mayores. Conclusión. Se observó una alta prevalencia de caries dental, edentulismo y enfermedades periodontales descritas en la literatura científica sobre la población quilombola de Brasil, lo que demuestra la necesidad de mejorar la atención dental prestada a ese grupo de la población.

OBJECTIVE: The objective of this systematic review with meta-analysis was to critically evaluate the available data on sensitivity and specificity of IHC compared with molecular tests in the detection of BRAF V600E mutation in ameloblastomas. MATERIALS AND METHODS: This systematic review was performed based on the PRISMA statement and registered in Prospero (CRD42021259117). PubMed, Web of Science, Scopus, and Cochrane Library databases were searched for observational studies to answer the question "What is the diagnostic accuracy of immunohistochemistry compared with molecular tests for the diagnosis of BRAF V600E mutation in ameloblastomas?". Methodological quality and risk of bias assessment of the selected studies were based on the QUADAS-2. Meta-analysis based on hierarchical SROC curve model and summary measures for sensitivity and specificity were computed. RESULTS: A total of 226 records were found, but only 05 articles met the inclusion criteria, with 277 FFPE specimens of ameloblastoma included in the quantitative analysis. The sensitivity of the IHC compared to molecular tests ranged from 0.71 to 1.00, while all of the included studies showed perfect specificity (1.00). Pooled measures for sensitivity and specificity were 0.95 [95% CI 0.89, 1.00] and 1.00 [95% CI 0.95, 1.00], respectively. The diagnostic odds ratio was 4.05, and the AUC for SROC curve was calculated as 0.979. CONCLUSIONS: BRAF V600E-specific IHC using VE1 antibody showed extremely high sensitivity and specificity when compared with molecular tests in the detection of the mutation in ameloblastomas.

Background: Oral Potentially Malignant Disorders (OPMDs) are defined as lesions with a greater likelihood ofprogressing to cancer. Population-based studies that evaluate the prevalence of OPMDs are scarce in Brazil. Theaim of the present study was to determine the prevalence of OPMDs and associated risk factors in a semi-urbanBrazilian population.Material and Methods: This is a cross-sectional study, whose universe included individuals aged 40 years or olderresiding in a medium-sized city of northeastern Brazil. Data collection was divided into two steps: interview andoral examination. The outcome variable was the presence of OPMDs. The predictor variables were sociodemo-graphic characteristics and risk habits. The bivariate analysis was performed through chi-square test. The crudeprevalence ratios (PR) and its respective 95% confidence intervals (CI) were calculated. Poisson regression analy-sis with robust variance was used to calculate adjusted PRs and 95% CI.Results: Three hundred fourteen individuals were included in the study. When asked about risk habits, 58.9%reported being current smokers or ex-smokers and 62.2% reported being current drinkers or ex-drinkers. Theprevalence of OPMDs was 7.6% and was significantly higher among individuals with black skin color (p < 0.001),alcohol users (p = 0.017), and individuals with both tobacco and alcohol habits (p = 0.012).Conclusions: Therefore, the population in the present study had a high frequency of risk habits associated withPMDs of the oral cavity.(AU)

Introdução: A Disfunção Temporomandibular (DTM) é a dor orofacial crônica mais comum, encontrada por dentistas e outros profissionais de saúde. Sua etiologia tem caráter multifatorial, e pode envolver, desde fatores hereditários, hábitos parafuncionais, maloclusões, até ansiedade e estresse. Objetivo: Este estudo investigou a frequência da DTM e a sua relação entre hábitos parafuncionais em uma população de pacientes classe II esquelética. Método: Uma amostra composta por pacientes classe II esquelética foi selecionada de forma não probabilística. Setenta e três indivíduos concordaram em participar da pesquisa, se submetendo ao exame físico que consistia na aplicação do Eixo I do (RDC/TMD). Resultados: Quarenta e seis por cento dos pacientes examinados apresentaram diagnóstico positivo. A média de idade dos pacientes foi de 27 anos+ 8,73 anos, 82% do gênero feminino e 80% brancos. Trinta e quatro pacientes referiram alguma atividade parafuncional. Desses a onicofagia representou o grupo com maior frequência, seguido pelo bruxismo, interposição de objetos entre os dentes e por último o apertamento. As variáveis não apresentaram associação estatisticamente significante com o desfecho primário estudado. Conclusão: Na amostra estudada não se observou associação entre a ocorrência de DTM e hábitos parafuncionais em pacientes classe II esquelética... (AU)


Introduction: Temporomandibular disorder (TMD) is the most common chronic orofacial pain, found by dentists and other health professionals. Its etiology has a multifactorial character, and may involve, from hereditary factors, parafunctional habits, malocclusions, to anxiety and stress. Objective: This study investigated the TMD frequency and its relationship among parafunctional habits in a population of skeletal class II patients. Method: A sample composed by a class II patients was selected in a non probabilistic manner. Seventy-three members agreed to participate in the research, submitting themselves to a physical examination that consists in the application of Axis I (RDC/TMD). Results: Forty-six percent of patients diagnosed with a positive diagnosis. The average age of the patients was 27 years ± 8.73 years, 82% female and 80% white. Thirty-four patients reported some parafunctional activity. Of these, biting nail represented the group most frequently, followed by bruxism, interposition of objects between the teeth and lastly clenching. As variables did not present a statistically significant association with the primary outcome studied. Conclusion: In the sample studied, it is observed that there is no association between TMD occurrence and parafunctional habits in skeletal class II patients... (AU)

Introdução: Os sialólitos são lesões mineralizadas nas glândulas salivares que causam obstrução total ou parcial do ducto, acometendo comumente a glândula submandibular. Sua abordagem varia de pouco invasiva à cirúrgicas, a depender do número, localização e dimensões dos cálculos. Objetivo: Esse estudo teve como objetivo relatar um caso clínico raro de sialólito no ducto da glândula parótida tratado através da remoção cirúrgica. Relato De Caso: Paciente compareceu ao ambulatório com história de dor e edema em face com 2 meses de evolução, referindo piora da sintomatologia após alimentação. Ao exame físico apresentou edema endurecido em região pré-auricular esquerda e ausência de drenagem no ducto da parótida ipsilateral. Foi realizado uma radiografia de tecidos moles com filme periapical, que revelou imagem radiopaca circunscrita sugestiva de um sialólito no ducto da glândula parótida esquerda. Assim, foi realizada excisão cirúrgica do cálculo seguida do reestabelecimento da patência ductal através da instalação de cateter venoso. Paciente evoluiu bem e segue em acompanhamento sem recidiva dos sinais e sintomas. Considerações Finais: O presente estudo revela que o diagnóstico precoce da sialolitíase e a escolha do plano de tratamento adequado estão associados a um bom prognóstico, e o reestabelecimento da patência ductal, quando danificado, é imprescindível para o sucesso do tratamento(AU)


Introduction: Sialoliths are mineralized lesions in the salivary glands that cause total or partial obstruction of the duct, commonly affecting the submandibular gland. It ranges from less invasive to surgical approach, depending on the number, location and dimension of the calculi. Objective: This study aimed to report a rare clinical case of a sialolith in the parotid gland's duct treated by surgical removal. Case Report: The patient attended the outpatient clinic with a history of pain and edema in the face with 2 months of evolution, reporting worsening symptoms after feeding. On physical examination, he had hardened edema in the left preauricular region and no drainage in the ipsilateral parotid duct. Soft tissue radiography with a periapical film was performed, which revealed a circumscribed radiopaque image suggestive of a sialolith in the left parotid gland's duct. Thus, the calculus's surgical excision was performed, followed by the reestablishment of the ductal patency through the installation of a venous catheter. The patient evolved well and is being followed up without recurrence of signs and symptoms. Final Considerations: The present study reveals that the early diagnosis of sialolithiasis and the choice of the appropriate treatment plan are associated with a good prognosis, and the reestablishment of ductal patency, when damaged, is essential for the success of the treatment(AU)


Introducción: Os sialolitos son lesiones mineralizadas en las glándulas salivales que causan obstrucción total o parcial del conducto, afectando comúnmente a la glándula submandibular. Su abordaje varía desde poco invasivo hasta quirúrgico, dependiendo del número, ubicación y dimensiones de los cálculos. Objetivo: Este estudio tuvo como objetivo reportar un caso clínico raro de sialolito en el conducto de la glándula parótida tratado mediante extirpación quirúrgica. Reporte de Caso: Paciente acudió a consulta externa con antecedente de dolor y edema en el rostro de 2 meses de evolución, refiriendo empeoramiento de la sintomatología tras la alimentación. A la exploración física presentaba edema endurecido en región preauricular izquierda y ausencia de drenaje en conducto parotídeo ipsilateral. Se realizó una radiografía de partes blandas con placa periapical, que reveló una imagen radiopaca circunscrita sugestiva de un sialolito en el conducto de la glándula parótida izquierda. Así, se realizó la escisión quirúrgica del cálculo seguida del restablecimiento de la permeabilidad ductal mediante la instalación de un catéter venoso. El paciente evolucionó bien y se le está dando seguimiento sin recurrencia de signos y síntomas. Consideraciones Finales: El presente estudio revela que el diagnóstico precoz de la sialolitiasis y la elección del plan de tratamiento adecuado se asocian a un buen pronóstico, y el restablecimiento de la permeabilidad ductal, en caso de daño, es fundamental para el éxito del tratamiento(AU)

OBJECTIVE: SARS-CoV-2 has high human-human transmission rate. The aerosols and saliva droplets are the main contamination source. Thus, it is crucial to point out that dental practitioners become a high-risk group of contagion by SARS-CoV-2. Based on this, protocols have been recommended to avoid cross-contamination during dental care; however, appropriate evidence has not yet been established. Our study sought to make a screening, by in silico analysis, of the potential of mouth rinses used in dental practices to prevent the dental workers' contamination by SARS-CoV-2. METHODOLOGY: Multiple sequence comparisons and construction of the phylogenetic tree were conducted using the FASTA code. Therefore, molecular docking investigation between SARS-CoV-2 proteins (Main Protease, Spike Glycoprotein, Non-structure Protein, and Papain-like Protease) and molecules used in dental practices (chlorhexidine digluconate, hydrogen peroxide, cetylpyridinium chloride, povidone-iodine, gallic acid, ß-cyclodextrin, catechin, and quercetin) was performed using AutoDock Vina. Moreover, 2D interactions of the complex protein-ligand structure were analyzed by Ligplot+. RESULTS: The obtained results showed a remarkable affinity between SARS-CoV-2 proteins and all tested compounds. The chlorhexidine digluconate, catechin, and quercetin presented a higher affinity with SARS-CoV-2. CONCLUSIONS: The overall results allowed us to suggest that chlorhexidine is the most suitable active compound in reducing the SARS-CoV-2 salivary load due to its better binding energy. However, in vivo studies should be conducted to confirm their clinical use.

The purpose of this systematic review was to summarize the available data on TMJ chondrosarcomas and to perform a survival analysis of cases reported to date. This review was conducted in accordance with the PRISMA. Two authors performed an electronic search of case reports of TMJ chondrosarcoma published until August 02, 2020. Forty-seven studies reporting 53 cases were included. Chondrosarcomas of the TMJ were more prevalent in women, with a male:female ratio of 1:1.4. Survival curves were significantly associated with histological diagnosis (p = 0.004), reconstructive surgery (p = 0.024), recurrence (p < 0.001), and distant metastasis (p = 0.001). Only distant metastasis was independently associated with survival (p = 0.017). TMJ chondrosarcomas presented with low recurrence and higher survival rates than other chondrosarcomas. Synovial subtype, absence of reconstructive surgery, and presence of local recurrence or distant metastasis were associated with poorer prognosis.

Background: The preemptive use of anti-inflammatory drugs, such as corticosteroids and NSAIDs, has the po-tential to reduce pain, swelling and trismus following oral surgery. The aim of this study was to compare theefficacy of dexamethasone and ketorolac tromethamine in reducing pain, swelling and trismus after mandibularthird molar removal.Material and Methods: The researches implemented a triple-blind, randomized clinical trial. The study was con-ducted with ASA I individuals aging between 18 and 35 years, which were randomized and submitted to twointerventions, one with 8mg dexamethasone and the other with 20mg ketorolac tromethamine given 1h before theprocedure. The primary predictor variable was the use of dexamethasone or ketorolac. The primary outcome vari-able was the postoperative pain level, measured with a Visual Analogue Scale. The secondary outcome variableswere the amount of rescue analgesic consumed, swelling and trismus. Repeated-measures ANOVA and t-test forpaired samples were used to compare the means. Significance was set at p < 0.05.Results: Fifty individuals were randomized and allocated to intervention, and the sample was composed of 40subjects who completed the study (27 female and 13 male). Dexamethasone, when compared to ketorolac trometh-amine, showed a significantly higher reduction in pain level at 8h, 16h, 24h, 32h, 40h and 72h, in swelling and tris-mus at 24h, 48h, 72h and 7 days and in total number of rescue analgesics taken up to 72h postoperative (p < 0.05). Abstract SARS-CoV-2 has high human-human transmission rate. The aerosols and saliva droplets are the main contamination source. Thus, it is crucial to point out that dental practitioners become a high-risk group of contagion by SARS-CoV-2. Based on this, protocols have been recommended to avoid cross-contamination during dental care; however, appropriate evidence has not yet been established. Objective Our study sought to make a screening, by in silico analysis, of the potential of mouth rinses used in dental practices to prevent the dental workers' contamination by SARS-CoV-2. Methodology Multiple sequence comparisons and construction of the phylogenetic tree were conducted using the FASTA code. Therefore, molecular docking investigation between SARS-CoV-2 proteins (Main Protease, Spike Glycoprotein, Non-structure Protein, and Papain-like Protease) and molecules used in dental practices (chlorhexidine digluconate, hydrogen peroxide, cetylpyridinium chloride, povidone-iodine, gallic acid, β-cyclodextrin, catechin, and quercetin) was performed using AutoDock Vina. Moreover, 2D interactions of the complex protein-ligand structure were analyzed by Ligplot+. Results The obtained results showed a remarkable affinity between SARS-CoV-2 proteins and all tested compounds. The chlorhexidine digluconate, catechin, and quercetin presented a higher affinity with SARS-CoV-2. Conclusions The overall results allowed us to suggest that chlorhexidine is the most suitable active compound in reducing the SARS-CoV-2 salivary load due to its better binding energy. However, in vivo studies should be conducted to confirm their clinical use.

Resumo Objetivo Estimar a prevalência do deficit cognitivo e verificar associações com variáveis sociodemográficas e de saúde em idosos de Unidades Básicas de Saúde (UBSs) de uma cidade no interior do nordeste brasileiro. Método Estudo transversal de prevalência e associação a respeito da população idosa. Os instrumentos utilizados para coleta dos dados foram Miniexame do Estado Mental (função cognitiva), Escala de Depressão Geriátrica Abreviada (sintomas depressivos), Escala de Lawton (capacidade funcional), Miniavaliação Nutricional (estado nutricional) e teste Timed Get Up and Go (risco de quedas). A associação bivariada entre o deficit cognitivo e as variáveis independentes foi avaliada pelo teste qui-quadrado. A análise multivariada foi feita usando um modelo de regressão logística com as razões de prevalência (RP) e intervalo de confiança de 95% (IC95%). Resultados Foram avaliados 818 idosos. A prevalência de deficit cognitivo foi de 65,9% (IC95%=62,50 - 69,10). Ao compor o modelo de análise multivariada, verificou-se maior ocorrência de deficit cognitivo em indivíduos mais velhos (RP=1,48; IC95%=1,07-2,05), funcionalmente dependentes (RP=3,27; IC95%=2,01-5,10), analfabetos (RP=1,66; IC95%=1,15-2,40) e com risco de desnutrição ou desnutridos (RP=2,09; IC95%=1,47-2,96). Conclusão O presente trabalho evidenciou alta prevalência de deficit cognitivo e que foi associada à idade, escolaridade, capacidade funcional e estado nutricional. É possível questionar se isso se deve à grande quantidade de pessoas com comprometimento cognitivo leve sem demência com remissão posterior dos sintomas, ou pela ocorrência de demência de início precoce.


Abstract Objective To estimate the prevalence of cognitive deficit and verify associations with sociodemographic and individual health variables in senior patients from units in the primary health system. Method Prevalence and association study about the elderly population. The instruments used were the Mini-Mental State Examination (cognitive function), Shorten Geriatric Depression Scale (depression symptoms), Lawton Scale (functional capacity), Mini-Nutritional Assessment (nutritional state), and Timed Up and Go Test (fall risk). The association between cognitive deficit and independent variables was verified by the Chi-square test. Multivariate analysis was performed using a logistic regression model with the prevalence ratio (PR) and 95% confidence interval (95%CI). Results The data were collected from 818 old-aged patients. The cognitive deficit had a prevalence of 65.9% (95%CI= 62.50-69.10). In the multivariate analysis model, it was verified a larger occurrence of cognitive deficit individuals, with risks of malnutrition (PR=2.09; CI95%=1.47-2.96), illiteracy (PR=1.66; 95%CI=1.15-2.40), dependents (PR=3.27; 95%CI=2.01-5.10), and with more than 70 years old (PR=1.48; 95%CI=1.07-2.05). Conclusion The present study showed a high prevalence of cognitive deficit and was associated with age, education, functional capacity, and nutritional status. It is possible to question if it is caused by the big amount of people with mild cognitive impairment without dementia with posterior remission of the symptoms, or by the occurrence of early start dementia.

Introdução: Tendo em vista a importância do campo da patologia oral e maxilofacial, faz-se necessário compreender o impacto da mais recente Classificação dos Tumores de Cabeça e Pescoço da OMS na frequência e distribuição dos cistos e tumores odontogênicos. Este estudo teve como objetivo estabelecer a frequência de lesões odontogênicas ao longo de 12 anos, em um serviço de referência em Patologia Oral, no Nordeste brasileiro. Metodologia: Os casos diagnosticados como cisto (OC) ou tumor (OT) odontogênico de 1999 a 2010 foram revisados e reclassificados de acordo com a atual Classificação da OMS de Tumores de Cabeça e Pescoço. Foram analisados dados referentes ao diagnóstico histopatológico, à localização da lesão, à idade, ao sexo e à etnia. A análise dos dados bivariados foi realizada, calculando-se as razões de prevalência, o teste do qui-quadrado e o teste exato de Fisher. Resultados: Entre 3.034 espécimes, 409 foram OC e 199 foram OT. Os Oc mais frequentes foram o cisto radicular (n = 129) e ceratocisto odontogênico (n = 99). Entre os OT, os mais frequentes foram ameloblastoma (n = 80) e odontoma (n = 47). Conclusões: Houve uma redução considerável na frequência relativa de OT após a reclassificação de lesões... (AU)


Introduction: Due to the importance of oral and maxillofacial pathology, it is necessary to understand the impact of the latest WHO Head and Neck Tumor Classification on the frequency and distribution of odontogenic cysts and tumors. This study aimed to establish the frequency of odontogenic injuries over 12 years in a reference service of Oral Pathology in the Northeast of Brazil. Methodology: All cases which received a diagnosis of odontogenic cyst (OC) or tumour (OT) from 1999 to 2010 were reviewed and reclassified according to the lastest WHO Classification of Head and Neck Tumors. Data regarding the histopathological diagnosis, location of the lesion, age, gender and Ethnicity were analyzed. The bivariate data analysis was performed by calculating the prevalence ratios, as well as the chi-square test and Fisher's exact test. Results: Among 3,034 specimens, 409 were OC and 199 were OT. The most frequent OC were the radicular cyst (n = 129) and odontogenic keratocyst (n = 99). Among the OT, the most frequents were ameloblastoma (n = 80) and odontoma (n = 47). Uma das principais causas de insucesso das reabilitações implanto suportada está relacionada à inflamação dos tecidos de sustentação peri-implantar a longo prazo. Mucosites podem ser definidas como alterações inflamatórias reversíveis dos tecidos moles periimplantares que não sofreram perda óssea. O conceito de mucosite alude a uma reação inflamatória de caráter reversível, sem perda óssea, equivalente à gengivite do periodonto. É caracterizada, principalmente, pela dor, sangramento gengival, eritema e ulcerações. A prevalência de complicações após a restauração oral com próteses implantosuportadas é alta. No entanto, existem poucos trabalhos na literatura sobre esse tipo de complicações, o que torna difícil comparar os resultados e avaliar se a presença de problemas protéticos é frequente ou não. The principal complication causes of failure of implant supported restorations is related to inflammation of the peri-implant tissues support the long term. Mucositis can be defined as reversible inflammatory changes in the peri-implant soft tissues that has not undergone bone loss. The concept of mucositis refers to an inflammatory reaction reversible character without bone loss, equivalent to periodontal gum disease. It is characterized by pain, gingival bleeding, erythema and ulceration. The prevalence of complications after oral restoration with implants supported dentures hybrids is high. However, there are few studies on this type of complications, which makes it difficult to compare results and assess whether the presence of prosthetic problems is frequent or not. Os cistos inflamatórios periapicais representam uma considerável parcela na distribuição epidemiológica daqueles categorizados como odontogênicos. Estudos mostram que no mundo inteiro cerca de 84% dos cistos que acometem a região maxilo facial são inflamatórios pariapicais. Seu diagnóstico é realizado pela associação entre o exame clínico, imaginológico e histopatológico. a terapêutica dessas lesões compreende desde o tratamento endodôntico dos dentes envolvidos até a sua enucleação cirúrgica. Fatores, como o estado geral do paciente, tamanho, forma e localização da lesão, são relevantes na tomada de decisões em casos como esses. Este trabalho tem como propósito relatar um caso clínico de um extenso cisto inflamatório periapical na maxila e discutir os fatores que levaram a uma abordagem multidisciplinar.


Inflammatory periapical cysts represent a considerable share in the epidemiological distribution of those categorized as odontogenic. Studies show that about 84% of cysts affecting the maxillofacial region in the world, are inflammatory pariapicais. Its diagnosis is made by the association between clinical examination, imaging and histopathology. The treatment of these injuries range from the endodontic treatment of teeth involved until his surgical enucleation. Factors such as the patient's general condition, size, shape and location of the injury are relevant in making decisions in such cases. This paper aims to report a case of an extensive inflammatory periapical cyst in the maxilla and discuss the factors that led to a multidisciplinary approach.
