RESUMEN

Mycotic involvement of viscera and nervous system develops in patients with immunodeficiency, during antibiotic treatment, and application of invasive methods of diagnosis and treatment, promoting penetration of the agent into the blood, which often results in development of mycotic foci in the visceral parenchyma and nervous system. The involvement is diagnosed from resistance to common therapy and is verified by laboratory investigations: microscopic examination of biological substrates, inoculation in nutrient media, serological tests, and histological analysis of biopsy specimens. Etiotropic therapy is carried out by antimycotic drugs with due regard for the agent sensitivity, drug pharmacokinetics, and drug tolerance. Antimycotic therapy should be carried out by a physician with at least the minimal knowledge of mycology.

RESUMEN

The base of current management of bronchopulmonary aspergillosis with allergic manifestations (BPAAM) is glucocorticosteroid treatment. These hormones are introduced in a mean dose 0.5 mg/kg (by prednisolone) to correct bronchial obstruction, reduce eosinophilia and resolve (completely or partially) infiltrates in the lungs. This being achieved, the dose is lowered to warrant maintenance. Minimal doses or discontinuation of glucocorticosteroids may be used in case of adjuvant indomethacin (0.025 three times a day) or plasmapheresis administration. In remission, fungistatic means with glucocorticosteroids or without them (in the absence of bronchial obstruction) are proper. Inhalations in BPAAM are ineffective. Individual schemes of the disease treatment may vary in wide range.

RESUMEN

Presents the findings of mycologic examinations of 12 patients with carcinomas of various sites. Though such examinations were carried out because of mycotic involvement of the mucous membranes of the digestive and respiratory organs, profound studies have revealed malignant tumors. A case history is presented. The authors come to a conclusion that candidiasis may complicate and mask the course of malignant tumors of various sites over all the phases of their development. This necessitates purposeful search for a possible tumor in patients with deep mycoses.

RESUMEN

It was established that most patients with non-Hodgkin malignant lymphomas against the background of secondary immunodeficiency show an activation of the saprophytic bacterial and mycotic flora that results in the development of different infectious complications essentially, thus, influencing the course and prognosis of the disease. A dependence was established between the malignancy grade of non-Hodgkins lymphomas and frequency of development of non-infectious complications.

RESUMEN

After prosthetics of the aortal valve 30 patients underwent bicycle ergometry training at the sanatorial stage of rehabilitation. 10 patients who did not undergo bicycle ergometry training made up the control group. The efficacy of bicycle ergometry training was assessed according to the time-course of changes in pump and contractile functions of the heart and oxygen consumption at rest and during bicycle ergometry. Transfer of the patients to a higher functional class, a decrease in the number of threshold reactions, attainment of a higher heart rate and a higher oxygen consumption, capacity for an increase of the stroke index up to the load of 1.0 W/kg and of the level of the myocardial reserve up to 1.5-2.0 W/kg were used as criteria for the effectiveness of bicycle ergometry training. The effectiveness of bicycle ergometry training is also supported by the data obtained on comparison of the findings in the main and control groups.

RESUMEN

Five patients (4 men and 1 woman) afflicted with bronchopulmonary aspergillosis (ABPA) were examined. The main criteria for the diagnosis were a high level of the total IgE, demonstration of specific IgE to Aspergillus fumigatus, positive skin tests with antigens of moldy fungi. All the 4 patients with active process, not given corticosteroids, manifested eosinophilia of the sputum and peripheral blood. The clinical picture of bronchial asthma was recorded in 2 patients. It is desirable that all the patients with protracted and relapsing infiltrative processes in the lungs running with sputum and blood eosinophilia undergo examinations for the presence of ABPA.
