PURPOSE: Tri-weekly carboplatin is an established neoadjuvant treatment for triple-negative breast cancer, enhancing pathological complete response (pCR) and overall survival. This study explores if weekly carboplatin provides lower toxicity and comparable pCR rates. METHODS/PATIENTS: A retrospective multicenter study (January 2021 to March 2023) compares outcomes of weekly and tri-weekly carboplatin. RESULTS: Among 104 participants, 60% received weekly and 40% tri-weekly treatments. Weekly administration had fewer discontinuations (56.5 vs. 70.7%, p = 0.154). Both schedules exhibited similar overall toxicity (p = 0.087), with slightly higher grade 3-4 toxicity in the tri-weekly group (56.1 vs. 48.4%, p = 0.126). Hematological toxicity was comparable, but the weekly group experienced more diarrhea (p = 0.432) and asthenia (p = 0.012). Weekly treatment correlated with more frequent breast-conserving surgeries (p = 0.004). pCR rates were 50% with weekly and 61% with tri-weekly regimens (p = 0.186). CONCLUSIONS: Weekly carboplatin exhibited comparable toxicity, a trend toward fewer interruptions, and similar pCR rates. Prospective studies are essential for validating these findings.

RESUMO Objetivou-se estimar a prevalência da Violência Sexual (VS) contra homens brasileiros e fatores associados. Estudo transversal, que estimou a prevalência de várias formas de VS entre homens brasileiros por meio de questionário virtual. Participaram do estudo 1.241 homens de todos os estados. A maioria era jovem, entre 18 e 39 anos de idade (61,7%), heterossexual (50,2%), do Sudeste (54,4%), das classes socioeconômicas C/D/E (72,1%) e brancos (64,3%). Sobre as prevalências de VS, 70,5% afirmaram ter sofrido VS sem contato, 43,1%, VS com contato, 23,9%, VS com penetração, e 33,1%, sexo forçado. Ser bissexual, ser homossexual e ser divorciado/separado aumentaram as chances de sofrer sexo forçado. Este estudo indica que homens bissexuais e homossexuais têm mais chances de sofrer VS, corroborando outras pesquisas. Entre divorciados/separados, as chances de sofrer VS também se mostraram significativas e precisam ser mais bem exploradas. O estudo oferece a possibilidade de problematização para acolhimento de homens vítimas de VS, pensando majoritariamente na prevenção de efeitos adversos após a violência e na implantação de políticas públicas da área da saúde mais direcionadas para o público-alvo, considerando os principais fatores associados.
ABSTRACT We aimed to estimate the prevalence of sexual violence against Brazilian men and associated factors. A cross-sectional study was conducted, which estimated the prevalence of various forms of sexual violence among Brazilian men by means of a virtual questionnaire. A total of 1241 men from all Brazilian states participated in the study. Most participants were young men between 18 and 39 years (61.7%), heterosexual (50.2%), from the Southeast (54.4%), from socioeconomic classes C/D/E (72.1%), and white (64.3%). Regarding the prevalences of sexual violence, 70.5% reported non-contact sexual violence, 43.1% sexual violence with contact, 23.9% sexual violence with penetration, and 33.1% forced sex. Being bisexual homosexual, and being divorced/separated increased the odds of experiencing forced sex. This study indicates that bisexual and homosexual men are more likely to suffer sexual violence, corroborating other research. Among divorced/separated men the chances of experiencing sexual violence were also significant and need to be further explored. The study offers the possibility of problematization for the care of male victims of sexual violence, focusing mainly on the prevention of adverse effects after violence and the implementation of public policies in health that are more directed to the target audience, considering the main associated factors.

Purpose: Based on our preview evidence that reduced nuclear content of the transcription factor Myc-associated protein X (MAX) is an early event associated with degeneration of retinal ganglion cells (RGCs), in the present study, our purpose was to test whether the overexpression of human MAX had a neuroprotective effect against RGC injury. Methods: Overexpression of either MAX or green fluorescent protein (GFP) in the retina was achieved by intravitreal injections of recombinant adenovirus-associated viruses (rAAVs). Lister Hooded rats were used in three models of RGC degeneration: (1) cultures of retinal explants for 30 hours ex vivo from the eyes of 14-day-old rats that had received intravitreal injections of rAAV2-MAX or the control vector rAAV2-GFP at birth; (2) an optic nerve crush model, in which 1-month-old rats received intravitreal injection of either rAAV2-MAX or rAAV2-GFP and, 4 weeks later, were operated on; and (3) an ocular hypertension (OHT) glaucoma model, in which 1-month-old rats received intravitreal injection of either rAAV2-MAX or rAAV2-GFP and, 4 weeks later, were subject to cauterization of the limbal plexus. Cell death was estimated by detection of pyknotic nuclei and TUNEL technique and correlated with MAX immunocontent in an ex vivo model of retinal explants. MAX expression was detected by quantitative RT-PCR. In the OHT model, survival of RGCs was quantified by retrograde labeling with DiI or immunostaining for BRN3a at 14 days after in vivo injury. Functional integrity of RGCs was analyzed through pattern electroretinography, and damage to the optic nerve was examined in semithin sections. Results: In all three models of RGC insult, gene therapy by overexpression of MAX prevented RGC death. Also, ON degeneration and electrophysiologic deficits were prevented in the OHT model. Conclusions: Our experiments offer proof of concept for a novel neuroprotective gene therapy for glaucomatous neurodegeneration based on overexpression of MAX.

Gangliosides are glycosphingolipids abundantly expressed in the vertebrate nervous system, and are classified into a-, b-, or c-series according to the number of sialic acid residues. The enzyme GD3 synthase converts GM3 (an a-series ganglioside) into GD3, a b-series ganglioside highly expressed in the developing and adult retina. The present study evaluated the visual system of GD3 synthase knockout mice (GD3s-/- ), morphologically and functionally. The absence of b- series gangliosides in the retinas of knockout animals was confirmed by mass spectrometry imaging, which also indicated an accumulation of a-series gangliosides, such as GM3. Retinal ganglion cell (RGC) density was significantly reduced in GD3s-/- mice, with a similar reduction in the number of axons in the optic nerve. Knockout animals also showed a 15% reduction in the number of photoreceptor nuclei, but no difference in the bipolar cells. The area occupied by GFAP-positive glial cells was smaller in GD3s-/- retinas, but the number of microglial cells/macrophages did not change. In addition to the morphological alterations, a 30% reduction in light responsiveness was detected through quantification of pS6-expressing RGC, an indicator of neural activity. Furthermore, electroretinography (ERG) indicated a significant reduction in RGC and photoreceptor electrical activity in GD3s-/- mice, as indicated by scotopic ERG and pattern ERG (PERG) amplitudes. Finally, evaluation of the optomotor response demonstrated that GD3s-/- mice have reduced visual acuity and contrast sensitivity. These results suggest that b-series gangliosides play a critical role in regulating the structure and function of the mouse visual system.

Introdução: A sífilis durante a gravidez é um grave problema de saúde pública. Em 2016, foram notificados 37.436 casos de sífilis em gestantes e 20.474 casos de sífilis congênita, com 185 óbitos no Brasil. O fator determinante para a diminuição dos casos é o pré-natal de qualidade. Objetivo: Avaliar o perfil dos casos notificados de sífilis em gestante no estado de Minas Gerais, entre 2013 a 2017. Métodos: Trata-se de um estudo transversal descritivo sobre o perfil epidemiológico dos casos notificados de sífilis em gestante em Minas Gerais entre 2013 a 2017. Os dados foram obtidos no Sistema de Informação de Agravos de Notificação, a partir do Departamento de Informática do Sistema Único de Saúde e categorizados quanto ao número de casos, ano de diagnóstico, faixa etária, nível de escolaridade, raça, região e estágio clínico ao diagnóstico. Resultados: Em 2017 foi demonstrado um aumento de 132% dos casos de gestantes com sífilis, em comparação com 2013. Houve predomínio na região central do estado (43,30%). 92,50% se referem a gestantes moradoras da zona urbana. A faixa etária mais acometida foi 20 a 39 anos. A maior parte (46,3%) se declara como parda. E a escolaridade 5ª a 8ª série incompleto do ensino fundamental foi a mais prevalente (16,05%). A classificação clínica mais comum da sífilis materna foi a primária (34,7%). Conclusão: A assistência pré-natal adequada deve ser iniciada precocemente. É necessária a capacitação das equipes das Unidades Básicas para garantirem ações educativas, identificação de situações de risco, diagnóstico e tratamento oportuno.
Introduction: Syphilis during pregnancy is a serious public health problem. In 2016, 37,436 cases of syphilis in pregnant women and 20,474 cases of congenital syphilis were reported, with 185 deaths in Brazil. The determining factor for the decrease in cases is the prenatal quality. Objective: To evaluate the profile of notified cases of syphilis in pregnant women in Minas Gerais, between 2013 to 2017. Methods: This is a cross-sectional descriptive study on the epidemiological profile of notified cases of syphilis in pregnant women in Minas Gerais from 2013 to 2017. Data were obtained from the Notification of Disease Information System, the from the Department of Informatics of the Unified Health System and categorized according to the number of cases, year of diagnosis, age group, level of education, race, region and clinical stage at diagnosis. Results: In 2017 there was a 132% increase in cases of pregnant women with syphilis compared to 2013. There was a predominance in the central region of the state (43.30%). 92.50% refer to pregnant women living in urban areas. The most affected age group was 20 to 39 years. Most (46.3%) declare themselves as brown. And the 5th to 8th grade incomplete elementary schooling was the most prevalent (16.05%). The most common clinical classification of maternal syphilis was primary (34.7%). Conclusion: Appropriate prenatal care should be started as early as possible. The training of Basic Units teams is necessary to ensure educational actions, identification of risk situations, diagnosis and timely treatment.

Glaucoma is a neurodegenerative disorder characterized by the progressive functional impairment and degeneration of the retinal ganglion cells (RGCs) and their axons, and is the leading cause of irreversible blindness worldwide. Current management of glaucoma is based on reduction of high intraocular pressure (IOP), one of its most consistent risk factors, but the disease proceeds in almost half of the patients despite such treatments. Several experimental models of glaucoma have been developed in rodents, most of which present shortcomings such as high surgical invasiveness, slow learning curves, damage to the transparency of the optic media which prevents adequate functional assessment, and variable results. Here we describe a novel and simple method to induce ocular hypertension in pigmented rats, based on low-temperature cauterization of the whole circumference of the limbal vascular plexus, a major component of aqueous humor drainage and easily accessible for surgical procedures. This simple, low-cost and efficient method produced a reproducible subacute ocular hypertension with full clinical recovery, followed by a steady loss of retinal ganglion cells and optic axons, accompanied by functional changes detected both by electrophysiological and behavioral methods.

BACKGROUND: Mesenchymal stem cells (MSCs) have been explored as promising tools for treatment of several neurological and neurodegenerative diseases. MSCs release abundant extracellular vesicles (EVs) containing a variety of biomolecules, including mRNAs, miRNAs, and proteins. We hypothesized that EVs derived from human Wharton's jelly would act as mediators of the communication between hMSCs and neurons and could protect hippocampal neurons from damage induced by Alzheimer's disease-linked amyloid beta oligomers (AßOs). METHODS: We isolated and characterized EVs released by human Wharton's jelly mesenchymal stem cells (hMSC-EVs). The neuroprotective action of hMSC-EVs was investigated in primary hippocampal cultures exposed to AßOs. RESULTS: hMSC-EVs were internalized by hippocampal cells in culture, and this was enhanced in the presence of AßOs in the medium. hMSC-EVs protected hippocampal neurons from oxidative stress and synapse damage induced by AßOs. Neuroprotection by hMSC-EVs was mediated by catalase and was abolished in the presence of the catalase inhibitor, aminotriazole. CONCLUSIONS: hMSC-EVs protected hippocampal neurons from damage induced by AßOs, and this was related to the transfer of enzymatically active catalase contained in EVs. Results suggest that hMSC-EVs should be further explored as a cell-free therapeutic approach to prevent neuronal damage in Alzheimer's disease.

Objetivo: Conhecer o itinerário terapêutico de pessoas com úlcera venosa crônica e as implicações para o cuidado de Enfermagem. Método: Pesquisa qualitativa desenvolvida em um centro de cuidados de Enfermagem. Participaram do estudo 11 pessoas com diagnóstico de úlcera venosa crônica. Os dados foram coletados entre agosto e setembro de 2016 por meio de entrevista semiestruturada, e analisados de acordo com a análise temática de Minayo. O estudo foi aprovado pelo Comitê de Ética em Pesquisas. Resultados: O itinerário terapêutico da pessoa com úlcera venosa crônica foi influenciado pelo saber popular, o apoio familiar e espiritual, além das relações com os serviços de saúde, que se mostraram fragmentados e nem sempre resolutivos. Conclusão: Evidenciou-se que estudos sobre itinerário terapêutico constituem-se como uma ferramenta para avaliar a qualidade da assistência de Enfermagem. Esta pesquisa teve como limitações poucas publicações na área da Enfermagem sobre essa temática
Objective: To know the therapeutic itinerary of people with chronic venous ulcer and the implications for nursing care. Method: Qualitative research developed in a Center of Nursing Care. Eleven people diagnosed with chronic venous ulcer participated in the study. Data were collected between August and September of 2016 through a semi-structured interview, and analyzed according to Minayo's thematic analysis. The study was approved by the Research Ethics Committee. Results: The therapeutic itinerary of the person with chronic venous ulcer was influenced by popular knowledge, family and spiritual support, as well as relationships with health services, which were fragmented and not always decisive. Conclusion: It was evidenced that studies on therapeutic itinerary constitute a tool to evaluate the quality of nursing care. This research had as limitations few publications in the area of nursing on this subject
Objetivo: Conocer el itinerario terapéutico de las personas con úlcera venosa crónica y las implicaciones para la atención de enfermería. Método: Investigación cualitativa llevada a cabo em un Centro de atención de enfermería. En el estudio participaron 11 personas con úlceras venosas crónicas. Los datos fueron recogidos entre agosto y septiembre 2016 a través de entrevistas semiestructuradas y analizados según el análisis temático de Minayo. El estudio fue aprobado por el Comité de Ética de la Investigación. Resultados: El tinerario terapéutico de las personas con úlcera venosa crónica fue influenciado por el conocimiento popular, el apoyo familiar y espiritual, además de las relaciones com los servicios de salud, que están fragmentados. Conclusión: Conocer que los estudios sobre itinerario terapéutico constituyen como una herramienta para evaluar la calidad de los cuidados de enfermería. Esta investigación tuvo el menor número de publicaciones limitaciones en el campo de la enfermería en este tema

Diabetic retinopathy is the leading cause of acquired blindness in working-age individuals. Recent work has revealed that neurodegeneration occurs earlier than vascular insult and that distal optic nerve damage precedes retinal degeneration and vascular insult. Since we have shown that optic nerve degeneration is reduced after optic nerve crush in Galectin-3 knockout (Gal-3 -/-) mice, we decided to investigate whether Gal-3 -/- could relieve inflammation and preserve both neurons and the structure of the retina and optic nerve following 8â¯weeks of diabetes. Diabetes was induced in 2-month-old male C57/bl6 WT or Gal-3 -/- mice by a single injection of streptozotocin (160â¯mg/kg). Histomorphometric retinal analyses showed no gross difference, except for a reduced number of retinal ganglion cells in WT diabetic mice, correlated to increased apoptosis. In the optic nerve, Gal-3 -/- mice showed reduced neuroinflammation, suggested by the smaller number of Iba1+ cells, particularly the amoeboid profiles in the distal end. Furthermore, iNOS staining was reduced in the optic nerves of Gal-3 -/- mice, as well as GFAP in the distal segment of the optic nerve. Finally, optic nerve histomorphometric analyses revealed that the number of myelinated fibers was higher in the Gal-3 -/- mice and myelin was more rectilinear compared to WT diabetic mice. Therefore, the present study provided evidence that Gal-3 is a central target that stimulates neuroinflammation and impairs neurological outcomes in visual complications of diabetes. Our findings provide support for the clinical use of Gal-3 inhibitors against diabetic visual complications in the near future.

A trauma to the mature central nervous system (CNS) often leads to persistent deficits, due to the inability of axons to regenerate after being injured. Increasing evidence suggests that pro-inflammatory and pro-apoptotic genes can present a major obstacle to promoting neuroprotection of retinal ganglion cells and consequently succeed in axonal regeneration. This study evaluated the effect of the absence of galectin-3 (Gal-3) on retinal ganglion cells (RGC) survival and axonal regeneration/degeneration after optic nerve crush injury. Two weeks after crush there was a 2.6 fold increase in the rate of cell survival in Gal-3-/- mice (1283±79.15) compared to WT animals (495.4±53.96). However, no regeneration was observed in the Gal-3-/- mice two weeks after lesion. Furthermore, axonal degeneration presented a particular pattern on those mice; Electron Microscopy (EM) analysis showed incomplete axon degeneration while the WT mice presented an advanced stage of degeneration. This suggests that the removal of the nerve fibers in the Gal 3-/- mice could be deficient and this would cause a delay in the process of Wallerian degeneration once there is a decrease in the number of macrophages/microglia in the nerve. This study demonstrates how the absence of Gal-3 can affect RGC survival and optic nerve regeneration/degeneration after lesion. Our results suggest that the absence of Gal-3 plays an important role in the survival of RGC and thus can be a potential target for therapeutic intervention in RGC neuroprotection.

A palavra gênero foi incluída no contexto social após a II Guerra Mundial em decorrência dos movimentos sociais feministas, que fundamentavam as distinções sociais relacionadas ao sexo biológico (do nascimento). Esses movimentos ganharam força na década de 1960 em função da desigualdade de poder entre o masculino e o feminino: preconizavam a mudança da condição feminina, sexo frágil, oprimido, submisso e excluído da sociedade. No campo das ciências da saúde, Robert Stoller, em 1968 no livro ?Sex and Gender?, introduziu a palavra gênero para diferenciar do termo sexo, que estava tão somente associado às condições biológicas. O termo gênero começou a fazer parte do Manual Diagnóstico e Estatístico dos Transtornos Mentais (DSM) da Associação Psiquiátrica Americana (APA), em 1994. O DSM-5 (2013) ampliou a visão sobre gênero e sexo: sexo refere-se tanto a masculino quanto a feminino, relacionado aos aspectos biológicos e de reprodução; gênero é utilizado para designar o papel social, menino ou menina, homem ou mulher e na maioria das pessoas relacionado ao sexo de nascimento. Entretanto, o desenvolvimento individual do gênero sofre influências biopsicossociais e nem todos os indivíduos perceber-se-ão como homens ou mulheres. Na atualidade os estudiosos do assunto questionam: o termo gênero está dissociado do sexo biológico? Há dois tipos de gêneros, masculino e feminino, somente? O gênero está ou não associado a fatores ambientais? O gênero é uma característica individual? Faz parte do corpo? É fundamental classificar o indivíduo em algum gênero? As experiências adquiridas ao longo do nosso desenvolvimento interferem na aquisição da identidade de gênero?

A expectativa de vida da população brasileira vem aumentando a cada ano, aumentando a demanda por tratamentos reabilitadores implantossuportados, trazendo benefícios para os pacientes incluindo melhora significativa na mastigação, auto-estima e qualidade de vida. Ao mesmo tempo, a idade do paciente pode ser considerado um fator de risco para o insucesso destes tratamentos. Embora o tratamento com implantes seja considerado um método previsível e seguro, são necessários conhecimentos sobre os critérios para indicação desta técnica, assim como os fatores limitadores locais e sistêmicos, condições de risco e complicações possíveis oriundas do tratamento. The life expectancy of the population is increasing every year, increasing demand for implant supported rehabilitation treatments, bringing benefits for patients including a significant improvement in chewing, self-esteem and quality of life. At the same time, the patients age can be considered a risk factor for the failure of these treatments. Although implant treatment is considered a predictable and safe method, knowledge is needed about the criteria for indication of this technique, as well as factors limiting local and systemic conditions of risk and possible complications from treatment. A saúde bucal é um compromisso essencial da sáude geral do trabalhador, influenciando diretamente o seu desempenho profissional. Em função dos programas de promoção de saúde no trabalho apresentarem um número limitado de ações específicas em saúde bucal, justificou-se a realização deste estudo pela necessidade de implantar um protocolo de promoção de saúde bucal junto às empresas. Com a implantação de um programa, evitaríamos que os distúrbios na saúde bucal do trabalhador prejudicassem sua eficiência, como também a da empresa, reduzindo o absenteísmo, trazendo um grande benefício para todos. Parece-nos claro concluir que as equipes de saúde bucal do trabalhador, devem nortear sua atuação para a promoção de saúde bucal por meio de um protocolo o qual facilitaria o trabalho das equipes por padronizar condutas de atuação no ambiente de saúde do trabalhador
