RESUMEN

To test the efficacy of detecting anti-Schistosoma mansoni cysteine proteinase antibodies (CP Abs) by cystatin capture (CC) ELISA in the diagnosis of prepatent schistosomiasis (before egg passing); 253 schistosome negative individuals were selected and divided into two groups. The first comprised 118 children whose first water contact occurred in March and April 1999 (primarily infected), and the second 135 individuals were previously treated for schistosomasis (re-infected). All the individuals were followed up triweekly by stool for detecting schistosome eggs and by serological tests for detecting antibodies against CP and anti-soluble egg antigens (SEA) by ELISA technique. CP seropositivity was detected in 92 from all examined individuals, out of them 38 were primarily infected (PI) children (20 of them were pre-patently treated), the rest; 54 were re-infected patients (28 out of them were pre-patently treated). The untreated (44) individuals from both groups were followed up till they passed eggs within 4 weeks and then were treated (post-patent). CP Abs were reassessed for the 92 patients after treatment, only 11 (12%) were still seropositive with marked decrease in optical density (O.D.) level than before treatment. Anti-SEA IgM Abs were sought in the 92 CP seropositive sera, and the seropositivity rate was lower in 38 PI children (5.3%) than in the 54 re-infected individuals (92.6%). The anti-SEA seropositivity rate in the PI children was 5% in the pre-patent and was 94.4% in the post-patent. None of the 161 CP seronegative individuals passed eggs up to 12 weeks.

RESUMEN

Anti-Strongyloides IgG antibodies were sought by conventional ELISA techniques among 115 immunocompromised and 36 patients parasitologically diagnosed to be infected by Strongyloides stercoralis through examining their GIT, and sputum specimens. Thirty six individuals infected with other helminths and 30 healthy parasitologically free individuals were also included. The specificity of the technique using thawed and sonicated Strongyloides larvae as an antigen was 100%. The sensitivity was respectively 92.2% and 13.3% among the immune-competent and immune-compromised. When anti-human biotinylated IgG and avidin-peroxidase conjugates were used, sensitivity among both groups rose to 100% and specificity remained unchanged. Avidin-biotinylated ELISA is recommended for the diagnosis of hidden and disseminated strongyloidiasis.

RESUMEN

A total of 190 patients were selected after being subjected to hemoculture for detecting Salmonella spp, coproscopy for Schistosoma mansoni eggs and ELISA testing for anti schistosomal sero-reactivity. They made up 4 group; The first comprised 31 febrile patients with positive hemoculture, coproscopy and ELISA, 13 of them with hepatic fibrosis and received combined anti- typhoid and antibilharzial therapy. The second comprised 53 febrile patient with the same criteria; 18 with hepatic fibrosis, and received anti typhoid & antibilharzial alternatively. The third group comprised 46 febrile patients with positive hemoculture and negative coproscopy but with positive ELISA 20 were hepatosplenic & received anti typhoid treatment. The fourth comprised 60 afebrile patients with negative hemoculture and positive coproscopy and ELISA 35 were hepatosplenic, and received anti-bilharzial treatment. 50 healthy individuals were selected as control. The controls and the patients; before and after treatment underwent investigation for determining levels of serum complement3 as a marker of immune complex precipitation activity and creatinine clearance, and urinary proteins excretion in 24 hours as measurements of glomerular function. The specific treatment either combined or alternative or single in the first 3 groups consequently showed marked improvement in the above criteria with insignificant additive pattern in the second. The fourth group showed non-significant difference in the above criteria after anti-bilharzial therapy among the hepatosplenic, meanwhile it improved these criteria significantly among the non-hepatosplenic--in the four groups--whose criteria were significantly higher than among the hepatosplenic before treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

250 patients with fever, hepatomegaly and eosinophilia were investigated for parasitic infection by stool, urine, thick blood smear examination and sero-diagnosis (ELISA for anti-Toxocara and IFA for anti-Fasciola). Those with patent parasitic infections (41) and anti-Toxocara seropositivity (10) were excluded from follow up. The other were 3 months followed up. The detected infections were 10 Ascaris, 15 Strongyloides, 20 Toxocara, and 25 Schistosoma mansoni. Other 30 were positive for Fasciola eggs. No other infections were found in the remained 109 patients. IFA anti-Fasciola antibody titres were significantly higher among Fasciola group than any other groups. It is concluded that IFA anti-Fasciola antibody titre greater than or equal to 1/256 with ethanol fixed antigen sections is of value in diagnosis of acute fasciolitic hepatic syndrome.

RESUMEN

Two different strains of Toxoplasma gondii were isolated from feline faecal deposits in two villages: Rommana (Northern Sinai) and El-Ekhewa (Sharkyia Governorate) as evidenced by their entirely different pathogenic effect and immune response in laboratory Swiss Webster mice. A man holding both villages as mutual residences, who was already proved to get acute toxoplasmosis twice made us avid to search for the presence of different strains in the two localities. The present study throws a suggestion that repeated Taxoplasma infection could be due to difference in strains.

Anti Toxoplasma antibodies were sought in 34 patients with uveitis and 20 healthy individuals by IHAT and IFAT. The total sero-positivity rate was 26.5%. The prevalence was significantly higher among patients with posterior (42.9%) than with anterior uveitis (7.1%) and control (10%) indicating significant role of Toxoplasma as an aetiologic agent of posterior uveitis. 14.4% of patients with seropositive uveitis were considered to be due to primarily acquired Toxoplasma infection as evidenced by their significant higher IFAT mean titre (576.7 +/- 322.1) than IHAT (48 +/- 18.5). The reported toxoplasmic anterior uveitis case is most probably secondary to posterior involvement.

Specific IgM anti-Toxoplasma antibodies were sought in 94 serum samples by means of D.S. IgM ELISA. These antibodies were detected in all cases of acute toxoplasmic lymphadenopathy, but up to 13 months from onset of symptoms and in out of 20 patients with chronic toxoplasmosis and in one out of 20 seronegative individuals. They were neither detected in samples from 4 infants with high IgG IFAT titres associated with hydrocephalus and cerebral calcification, nor in samples from patients containing rheumatoid factors and those with lymphoma or infectious mononucleosis. Thus the detection of IgM antibodies seems non decisive mean for identifying infected infants and for determination of the exact date of acquiring the infection.
